Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 ` <br />Inspection date - October 28 and 29, 2003 <br />violation was corrected during the inspection on May 17, 2001. Two additional violations were <br />issued subsequent to the inspection as class two violations. One violation was for violating <br />California Code of Regulations, section 66264.13(a), for failure to conduct a detailed waste analysis <br />of spent parts washer. The second violation was for violating California Code of Regulations, <br />section 66264.73(a)(b) in that the facility failed to provide, upon request, a waste analysis and <br />waste determination. The facility was returned to compliance on Dec. 12, 2001. <br />During the 2001/2002 fiscal year, on August 16, 17, 2001, a CEI and hauler inspection was <br />conducted. No violations were discovered during the inspection. A Summary of Observations <br />issued. <br />On November 20, and 21, 2002, a CEI inspection was conducted. A hauler inspection was not <br />conducted. Previous to this inspection it was determined that the haulers registration was not <br />issued to LLNLS300 but to the LLNL Main Site in Livermore. LLNLS300 does not operate as a <br />transporter and does not have a hauler registration. No violations were discovered during the <br />inspection. A Summary of Observations issued. <br />IV.DOCUMENTS REVIEWED <br />a. Manifests, Bills of Lading LDR's and Exception Reports: <br />Prior to conducting the inspection I did a search of the Department's manifest data base and <br />downloaded several manifests to compare with the facility files. The following manifests were <br />reviewed: 21766507, 21766501, 21766552, 21766553, 21766563, 21766565, 21766571, 21766582, <br />21766589, 21766590, 21766523, 21766516, 21766492. According to Mr. Tageson there have been <br />no exception reports filed since the last inspection conducted on November 20 - 21, 2002. <br />Manifest 21766492 (Attachment D) was lacking the handling codes in section K of the manifest. It is <br />the responsibility of the TSDF' to place the handling codes in section K of a manifest to indicate to the <br />generator what was done with the waste.
