Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />On December 8, 2003, 1 phoned LLNLS300 and spoke with Stan Terusaki. I told him that LLNLS00 <br />would need to provide the Department with a training plan, which would include all the elements <br />required pursuant to California Code of Regulations, section 66164.16. 1 added that this should be <br />relevant to the personnel doing the inspections of Bldg. 829, the inspection plan and inspection logs <br />to be used during the inspections and how to document observations made during the inspection. I <br />added that the information should include the training requirements, course content, frequency, and <br />who is required to take this training with respect to conducting the required inspections of Bldg. 829 <br />and the area covered by the Post -Closure permit. <br />d. Incident Report: <br />None. <br />e. Waste Analysis Plan and Records: <br />I conducted a brief review of the Waste Analysis Plan (WAP) for EWSF, Bldg. 883. This plan is much <br />the same as the WAP for EWTF, Bldg. 845-A/B OD/OB. I also reviewed a waste profile representative <br />of waste determination by generator knowledge. No violations were observed. <br />f. Operation Log_ <br />I reviewed the operating logs for EWTF, Bldg. 845-A/B OD/OB. The OD unit, Bldg. 845-A, treats form <br />one waste only. The last treatment was of six pounds of explosive waste, which was detonated on May <br />13, 2003. Detonation activity does not often generate ash. Generally the only thing left after a <br />detonation is fragments of the container which held the explosive being treated. <br />The last date of burn activity for 845-B OB was on October 23, 2003. The waste treated was form <br />three. This area can treat form two, form three and form four waste. Detailed information on the waste <br />treated at LLNLS300 may be found in the. EWTF Operating Plan and in the Waste Analysis plan kept <br />on file at the Department's Berkeley office. <br />Operating logs for M-816 and M-2 were also reviewed. I asked for and received a copy of the inventory <br />for Bldg. 883 (Attachment 1). I also reviewed operating logs for Bldg. 883. 1 asked for and received <br />a copy of the inventory for Bldg. 883 (Attachment G). The inventory logs record various types of <br />information about the waste received. That information includes the date the waste entered the facility <br />
