Inspection Report Lawrence Livermore National Laboratories Site 300 Inspection date - October 28 and 29, 2003 After leaving Bldg. 829 it was decided to go on over to Mr. Tageson's office and start my document review with the manifests. Mr. Tageson maintains the facility manifest files in his office. The document review was completed the following day on October 29, 2003. For details and results of the manifest review, see Section IV of this report. Before leaving for the day a short informal close out meeting was held. October 29, 2003 Upon arrival at LLNLS300 I was met by Mr. Terusaki and Dawn Chase. A short in -briefing was held. I again explained the purpose of the inspection and what it would consist of. I then asked for and received consent to continue my inspection from Ms. Chase. The remainder of the day was spent completing my document review. Details and results of the documents reviewed may be found in Section IV of this report. Before completing the document review Ms. Folks arrived. I mentioned to Nts. Folks that I had failed to obtain a photograph of well W-829-1938 that is located just outside the capped area. She suggested that they get the photograph for me and e-mail it to me instead of making another trip out there which would include waiting to determine if a shot were going to be made and whether or not it would be clear for us to go into that area. I said that would be fine. I received the e-mailed copy of the digital photograph of the well on November 4, 2003 (Attachment J, photograph 3). The e-mail has been included with this report as an attachment (Attachment K). VINIOLATIONS Summary of Observations attached?
