Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />After leaving Bldg. 829 it was decided to go on over to Mr. Tageson's office and start my document review with <br />the manifests. Mr. Tageson maintains the facility manifest files in his office. The document review was <br />completed the following day on October 29, 2003. For details and results of the manifest review, see Section <br />IV of this report. Before leaving for the day a short informal close out meeting was held. <br />October 29, 2003 <br />Upon arrival at LLNLS300 I was met by Mr. Terusaki and Dawn Chase. A short in -briefing was held. I again <br />explained the purpose of the inspection and what it would consist of. I then asked for and received consent to <br />continue my inspection from Ms. Chase. The remainder of the day was spent completing my document <br />review. Details and results of the documents reviewed may be found in Section IV of this report. <br />Before completing the document review Ms. Folks arrived. I mentioned to Nts. Folks that I had failed to obtain <br />a photograph of well W-829-1938 that is located just outside the capped area. She suggested that they get <br />the photograph for me and e-mail it to me instead of making another trip out there which would include waiting <br />to determine if a shot were going to be made and whether or not it would be clear for us to go into that area. I <br />said that would be fine. I received the e-mailed copy of the digital photograph of the well on November 4, <br />2003 (Attachment J, photograph 3). The e-mail has been included with this report as an attachment <br />(Attachment K). <br />VINIOLATIONS <br />Summary of Observations attached? Yes (Attachment A) <br />Subsequent to the inspection a violation has been determined to exist. That violation is listed below. <br />There were no Class 1 or Class 2 violations observed. <br />Minor Violation <br />1.- LLNLS300 violated California Code of Regulations, section 66263.16, in that on or before October 29, <br />
