LAWRENCE LIVERMORE NATIONAL LABORATORY -SITE 300 Hazardous Waste Facility <br />EPA ID No. CA 2890090002 Post -Closure Permit <br />Effective Date: April 3, 2003 Page 5 of 13 <br />(g) In case of conflicts between the Operation Plan and the Permit, the Permit <br />conditions take precedence. <br />(h) This Permit includes and incorporates by reference any conditions of <br />waste discharge requirements issued by the State Water Resources <br />Control Board or any of the California Regional Water Quality Control <br />Boards and any conditions imposed pursuant to section 13227 of the <br />Water Code. <br />3. COMPLIANCE WITH CALIFORNIA ENVIRONMENTAL QUALITY ACT (CEQA) <br />A Negative Declaration for this project has been prepared in the accordance with <br />the requirements of Public Resources Code section 21000 et seq. and the <br />CEQA Guidelines, CCR, title 14, section 15070 et seq. <br />4. ENVIRONMENTAL MONITORING <br />a) The Permittee is required to continue to perform groundwater monitoring <br />in accordance with the requirements of CCR, title 22, division 4.5, chapter <br />14, article 6 and as specified in Operation Plan. Figure 3 of this Permit <br />shows goundwater monitoring well locations. Table 1 lists components of <br />groundwater constituesnts of concern (CDCs), analytical methods, and <br />concentration limits. Monitoring frequency shall be as follows: chemical <br />analyses of groundwater of all COCs and measurement of groundwater <br />elevations on a quarterly basis for the proposed new monitoring well and <br />existing two monitoring wells, 829-15 and 829-22 for the first year of the <br />post -closure period. After the second year, the frequency shall change to <br />semi-annually for groundwater elevations and annual monitoring for all <br />laboratory parameters and other field parameters if evaluation of the data <br />justifies a longer time period between monitoring events. DTSC approval <br />shall be required for any such change. Should the list of monitoring <br />parameters be reduced in the future, the original list of monitoring <br />parameters shall be analyzed every five years. DTSC approval shall be <br />required for any such change. <br />b) A visual inspection of the covered area shall be conducted, at a minimum, <br />on a monthly basis. Additional inspections shall be performed after 25 <br />year 24 hour rainstorms, and after a significant earthquake (see Part <br />4(d)), or other events which may cause substantial damage to the capped <br />Facility. On a yearly basis, the Permittee shall conduct a comprehensive <br />inspection as detailed in the Operation Plan. An independent engineer <br />registered in California shall approve the evaluation. All inspections shall <br />be conducted in accordance with Section 6.1, of the Operation Plan. <br />
