LAWRENCE LIVERMORE NATIONAL LABORATORY -SITE 300 Hazardous Waste Facility <br />EPA ID No. CA 2890090002 Post -Closure Permit <br />Effective Date: April 3, 2003 Page 7 of 13 <br />A: Within 60 days of the effective date of this permit, the Permittee shall <br />submit to DTSC for review and approval a groundwater monitoring <br />well design and monitoring plan. <br />B: Within 90 days of DTSC's approval of the groundwater monitoring -well <br />design and monitoring plan, the Permittee shall install the subject well. <br />C: Within 60 days of completion of well installation, the Permittee shall <br />submit to DTSC as -built drawings. <br />D: The Permittee shall conduct the sampling and analysis of this additional <br />groundwater monitoring well in accordance with the requirements <br />listed in Table 1 and with the post -closure permit application dated <br />December, 2001. <br />P <br />
