DTSC'S PREFERRED FORMAT FOR MANIFEST CORRECTION LETTERS (MCS <br />A Manifest Correction Letter must be submitted to the Department of Toxic Substances Control <br />(DTSC) whenever Uniform Hazardous Waste Manifests (Manifests) are submitted containing <br />incorrect or incomplete information such as EPA ID numbers, addresses, etc. DTSC's preferred <br />format for MCLs is a single page (if possible) letter on company letterhead (if available) listing <br />the following information for each incorrect manifest: <br />1. State Manifest Document Number. This is the eight -digit number pre-printed in Item A. <br />and also along the left border of the Manifest. <br />2. Ship Date. This is the date the Generator signed the Manifest in Item 16. <br />3. Generator's U.S. EPA ID Number. List the Generator's U.S. EPA ID Number used on the <br />original Manifest(s) even if itis incorrect. <br />4. Indicate incorrect or incomplete ItetNe Manifest(s). the Item Number(s) 1-20 and/or <br />A -K that are incorrect or incomplete <br />5. Correction(s). Supply the corrected and/or completed information in the MCL. Please do <br />not refer to attached corrected copy(s) of Manifest(s). <br />6. Signature. With Signator's printed name, title, mailing address, and phone number. <br />An individual MCL is not necessary for each incorrect <br />foManifest. OnfeMCL that <br />ast. Please do not attach copies of Manifests to MCLs. <br />If the incorrect information is simply a discrepancy between the quantity or type of waste <br />designated on the Manifest, and the quantity or type of waste the Treatment, Storage, or <br />Disposal Facility (TSDF) actually receives, an MCL is not necessary.
