Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />Terusaki regarding his presence during inpsection <br />Mr. Pat Gallagher, Explosive Technologist, EWSF <br />Mr. H. Dean Adams, Explosive Technician, EWTF, Bldg. 845A/B <br />Mr. White <br />Type of Business: LLNL/S300 is owned by the U.S. Department of Energy (DOE) and jointly operated <br />by the University of California and DOE. LLNL/S300 is a field test facility for <br />theoretical and developmental work and has been used since 1953 for testing non- <br />nuclear HE compounds and for conducting particle beam research. <br />II. CONSENT <br />Consent to conduct inspection that involves: taking photographs, reviewing and copying records, <br />questioning personnel and inspecting hazardous waste handling areas. <br />Consent given by: Mr.James Lane, Site 300 Deputy Manager <br />III. DOCUMENTS REVIEWED <br />a. Manifests, Bills of Lading, LDR's and Exception Reports: <br />I began my document review at the end of the day on October 28, 2003 with the manifest review. <br />No exception reports have been filed since the last inspection. Prior to conducting the inspection I did a <br />search of the Departments Data base and downloaded several manifests to compare with manifests in <br />the facility files. The following manifests were reviewed: 21766507, 21766501, 21766552, 21766553, <br />21766563, 21766565, 21766571, 21766582,21766589,21766590, 21766523, 21766516, 21766492. <br />Manifest 21766492 (Attachment C) was lacking the handling codes in section K of the manifest. I <br />brought this to Mr. Tageson's attention. Mr. Tageson explained that the specific waste stream received <br />was stored in Magazine 816 prior to treatment in the permitted OB/OD 845A -B. He stated that the <br />manifests for waste to be treated go to Mr. John Scott, who is the facility manager for EWTF, and <br />Magazine 816, and Magazines one through five.ca/I Stan Terusaki to clarify Mr. Scott's <br />responsibilities w/respect to offsite manifests, and the exact name of the _group that manages <br />manifests in the main site. He added that the manifests ultimately go back to the main LLNL in <br />DTSC (3/95) Five 3 of 12 <br />
