• Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />Livermore and from there the required copies are sent on to the generator and to the Department. <br />During the exit meeting on October 29, 2003, 1 discussed this manifest issue with Mr. Lane, who <br />acknowledged that there is a need for better communications the manifest management group located <br />at the main site in Livermore <br />b. Contingency Plan: <br />Reviewed the contingency plans for EWSF and EWTF and Bldg. 829. No violations were observed. <br />c. Training Plan and Records: <br />Reviewed training plan and training records. Reviewed training records for Mr. Greg Walter, EWTF <br />Technician Check with Stan Terusak re_gardin_g exact work title, Mr. Patrick Gallagher, Explosive <br />Technologist, EWSF, who is responsible for managing hazardous waste at Magazine 816 and <br />Magazines one through five, which includes conducting the required inspections. Mr. Walter has <br />responsibilities for managing hazardous waste and conducting inspections of the permitted drum <br />storage unit Bldg. 883. All training records reviewed were found to be in order. No violations were <br />observed. <br />d. Incident Report: <br />None. <br />e. Waste Analysis Plan and Records: <br />Conducted a brief reviewed of the Waste Analysis Plan (WAP) for EWSF, Bldg. 883. This plan is much <br />the same as the WAP for Bldg. 845A/B, the permitted treatment facilities. I also reviewed a waste <br />profile representative of waste determination by generator knowledge (Attachment ###) Ask Stan <br />Terusaki to fax the copy of that profile to me) . waste streams being stored in the permitted drum <br />storage Bldg. 883. No violations were observed. <br />f. Operation Log: <br />Reviewed logs for EWTF, 845A -B. The last detonation at Bldg. 845-A was on May 13, 2003. Six (.06— <br />Check w/Stan Terusaki for affirmation that this represent 6pounds and not 6 100's of a pound or <br />6 tenths of pound) pounds of form one waste was treated at that time.
