Inspection Report <br />on hazardous waste manifest number 23440668 for treatment at Safety Kleen in Oakland. On or before February <br />10, 2005, Safety Kleen Oakland notified LLNL Site 300 that they could not accept the shipment of hazardous waste. <br />On February 10, 2005, LLNL Site 300 picked up the rejected load of hazardous wastes at Safety Kleen in Oakland <br />under new hazardous waste manifest number 23440682 and delivered the rejected load of hazardous waste to an <br />alternate TSDF facility, Clean Harbors located in San Jose. <br />Corrective Action: <br />Within 7 days of receipt of this report, LLNL Site 300 shall comply with the. rejected load regulations specified in <br />Health and Safety Code section 25160.6. <br />2- LLNL Site 300 violated Health and Safety Code section 25189 (a) in that on or about February 10, 2005, LLNL <br />site 300 made, a false representation on hazardous waste manifest number 23440682. According to manifest <br />number 23440682 on February 10, 2005, LLNL Site 300 released 7 containers of hazardous waste to LLNL as <br />transporter to Clean Harbor of San Jose. Based on LLNL Site 300 documents and manifest 2340668, these same <br />7 containers were transported by Safety Kleen to Safety Kleen of Oakland on January 31, 2005. Upon arrival, the 7 <br />containers of hazardous waste to Safety Kleen Oakland rejected the 7 containers. The 7 containers remained at <br />Safety Kleen Oakland from January 31, 2005 until picked up for transporter by LLNL on February 10, 2005. <br />Corrective Action: <br />Within 7 days of receipt of this report, LLNL Site 300 shall comply with the rejected load regulations specified in <br />Health and Safety Code section 25160.6. <br />Unresolved issues since October 28, and 29, 2003, Compliance Evaluation Inspection (CEI) <br />LLNL Site 300 violated California Code of Regulations, title 22, section 66264.16 in that on or before October 28, <br />2003, the facility failed to have a training plan as part of the Part B, Operating Plan for the post -closure unit Building <br />829. The facility response to the violation is currently under review by the Department. <br />VII. Discussions with Operator: <br />June 21, 2005: <br />On June 21, 2005, after completing my inspection, I held an exit meeting with the facility representatives. I issued <br />Summary of Observation (SOO). The SOO was signed by Ms. Folks. See attachment N I also discussed <br />observations I made during the inspection with the facility representatives. I did identify an issue pertaining <br />hazardous waste manifest number 23440668. The hazardous waste manifest showed that LLNL S 300 shipped <br />seven containers of Non-RCRA hazardous waste, liquid (ethoxylted alcohols, trimethylhexanoic acid (MPC) on <br />Page 18 of 20 <br />
