Violation #1: <br />Within 30 days of receipt of this report, submit biennial reports for 1990 and 1992 to the appropriate <br />State agency and retain copies onsite for inspection. <br />Violation #2: <br />Effective immediately, keep copies of all hazardous waste manifests on site for three years. <br />Violation #3: <br />Within 30 days of receipt of this report, provide adequate spill and decontamination equipment at all <br />hazardous waste storage areas. <br />Violation #4: <br />Effective immediately, the facility contingency plan shall be available for emergencies and inspections. <br />Violation #5: <br />Within 15 days of receipt of this report, prepare employee safety training plan and train employees <br />regarding hazardous waste according to specifications outlined in code sections stated. <br />Violation #6: <br />Within 30 days of receipt of this report, existing hazardous waste storage tank must be monitored for <br />leakage and evaluated for integrity as outlined in code sections stated. <br />When the violations have been corrected, please send written certification to this office at the <br />letterhead address.
