• RECEIVEL <br /> The following is an itemized list of hazardous waste violations that have not been 19 2015 <br /> addressed for AMERICAN TIRE & TRUCK REPAIR as of February 117, 2ENVIRONMENTAL <br /> WCAITL nr.:DADTAACKrr <br /> Open violations from October 15,2014 inspection <br /> Violation #112 -Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for the last three years were not found on site. No records were found t <br /> disposal of used antifreeze For disposal of used oil, there were three records from 2014, one from 2013, and one fr+ <br /> 2012. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br /> receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br /> review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and <br /> submit copies to the EHD. This is a minor violation. <br /> Violation#114- Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. <br /> copy of each bill of lading must be kept on site for at leastthree years. Immediately locate a copy of all bills of lading <br /> or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD. <br /> This is a Class 11 violation. <br /> Violation#117 - Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers,cafepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. This is a Class II violation. <br /> Violation##301 - Facility not malntained to minimize the release of a hazardous waste. <br /> Black staining was observed on the ground in the parking lot in front of the facility, around the used antifreeze tank ai <br /> the surrounding buckets and containers which previously held oil and used oil. Black staining was observed in vara <br /> parts of the asphalt surrounding the facility, and on the dirt west of the south storage building. Black, oily staining <br /> was observed around the storm drain in the asphalt_ According to Mr. Bassi, he uses this drain to rinse the soapy <br /> water used on the used tires. According to Mr. Bassi, the storm drain flows to the southeast portion of the yard; dark <br /> liquid was observed accumulated in this area, Facilities shall be maintained and operated to minimize the possibility <br /> of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or <br /> the environment. Immediately clean stained areas, and manage according to Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. This is a repeat <br /> violation, Class II. <br /> Violation#302 -Failed to malntaln required emergency equipment. <br /> Spill control equipment was not found on site, According to Mr_ Bassi, he used to have kitty litter to clean up spills, <br /> but no longer has any on site. All facilities shall be equipped with an internal communication or alarm system, a <br /> device capable of calling outside emergency help, spill control equipment, and decontamination equipment at all <br /> times. Immediately provide spill control equipment and maintain on site. This is a minor violation. <br /> Violation#403 -Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> A 5 gallon blue plastic bucket containing oily water was observed in the parking lot in front of the facility without a lid. <br /> A 5 gallon blue plastic bucket containing a pourable amount of used oil, and two drain pans of used oil were observe, <br /> open In the shop, according to Mr. Bassi, the drain pans were from oil changes earlier this week. A black plastic <br /> container and clear plastic container of oily water were observed in the yard behind the shop without lids. All <br /> hazardous waste containers shall be closed at all times except when adding or removing waste, Immediately close <br /> these containers, or place the wastes in properly closed containers, and ensure all hazardous waste containers are <br /> closed when not adding or removing waste.
