EMVEM� <br /> MAY 19 2015 <br /> ENVIRONMENTAL <br /> HEW TL_f r)[:DAOTAACKIT <br /> The following is an itemized list of hazardous waste violations that have not been <br /> addressed for AMERICAN TIRE & TRUCK REPAIR as of February 17, 2015. <br /> Open violations from October 15, 2014 inspection <br /> Violation#406 -Contaminated container not marked with date emptied or managed within one year of <br /> being emptied. <br /> A 15 blue steel drum which previously held gear oil was observed in the southeastyard with no empty labeling. A 20 <br /> gallon white steel drum was observed in the steel shed with no empty labeling; according to Mr. Bassi, the steel shec <br /> is where he stores all of his containers of oil and other products once they arm empty, Containers larger than five <br /> gallons that previously held a hazardous material shall be marked with the date they were emptied and managed <br /> within one year of being emptied. Immediately mark and manage all empty containers per this sectlon. This is a <br /> repeat violation, Class II. <br /> Violation#605 -Failed to completely label containers or tanks of hazardous waste. <br /> A 5 gallon blue plastic bucket containing oily water was observed in the parking lot in front of the facility without a <br /> label. A 5 gallon blue plastic bucket containing a pourable amount of used oil, and two drain pans of used oil were <br /> observed unlabeled in the shop; according to Mr, Bassi, the drain pans were from oil changes earlier this week. A <br /> black plastic container and clear plastic container of oily water were observed in the yard behind the shop without <br /> labels. A 55 gallon steel drum containing water mixed with grease was observed in the yard west of the south storag <br /> building without a label. All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> hazardous properties(ignitable, corrosive, reactive, toxic) <br /> physical state (liquid, solid) <br /> -composition (contents) <br /> accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required informatlon_ SincE <br /> it cannot be determined how long these containers have been on site, immediately contact a licensed hazardous <br /> waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. This is a repeat <br /> violation, Class II_ <br /> Violation 4608 -Failed to properly manage used oil and fuel filters. <br /> Two used oil filters were found In an open, unlabeled black plastic container in the yard behind the facility, Used oil <br /> filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as'"drained used oil filters," <br /> and marked with an accumulation start date. Immediately containerize and label these filters, and ensure that all <br /> used oil filters are managed in this way. This is a repeat violation, Class 11. <br /> I_M] <br />
