... <br />.., <br />The following is an itemized list of hazardous waste violations that have not been <br />addressed for PAULS AUTO REPAIR as of June 08, 2016. <br />Open violations from January 26, 2016 inspection <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, <br />transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web si <br />http://www.dtsc.ca.gov/IDManifestIPERMHWID.cfm for the form to reactivate the ID number. Immediately reactivate <br />your EPA ID number and submit evidence to the EHD. <br />Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for January 2013 through January 2016 were not <br />found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br />all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br />review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and <br />submit copies to the EHD. <br />Violation #112 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for January 2013 through January 2016 were not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts use <br />in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediate <br />locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br />EHD. <br />Violation #116 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on September 5, 2013 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by October 5, 2013. This <br />information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Violations not <br />resolved: Inactive EPA ID number. <br />Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br />hazardous waste. <br />Containers of hazardous waste open: <br />- Two white poly 55 gallon drums of used oil on the south side of the facility <br />- Two white poly 55 gallon drums of used oil on the west side of the facility <br />- One blue 55 gallon drum of used antifreeze on the south side of the facility <br />- A silver container with used oil spills used as secondary containment on the south side of the facility <br />- A silver container with used oil spills used as secondary containment on the south side of the facility <br />All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately clos <br />these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br />Violation 9406 - Contaminated container not marked with date emptied or managed within one year of <br />being emptied. <br />Two empty white poly 55 gallon drums on the west side of the shop had an empty date of 11-8-13 and were not <br />managed within a year .Containers larger than five gallons that previously held a hazardous material shall be markec <br />with the date they were emptied and managed within one year of being emptied.
