18,01 <br />The following is an itemized list of hazardous waste violations that have not been <br />addressed for PAULS AUTO REPAIR as of June 08, 2016. <br />Open violations from January 26, 2016 inspection <br />Violation #601 - Stored hazardous waste onsite greater than 180 days. <br />A blue 55 gallon drum of used antifreeze had a start accumulation date of 3-415. Facilities who generate less than <br />1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store <br />waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under <br />manifest and submit a copy of the manifest to the EHD. <br />Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br />Containers of hazardous waste not properly labeled: <br />- Two white poly 55 gallon drums of used oil on the south side of the facility did not have a start accumulation date <br />- Two white poly 55 gallon drums of used oil on the west side of the facility did not have a start accumulation date <br />- A silver container oil spills used as secondary containment on the south side of the facility did not have a hazardous <br />waste label <br />- A silver container oil spills used as secondary containment on the south side of the facility did not have a hazardouE <br />waste label <br />- A white 5 gallon container of metal brake shavings did not have a hazardous waste label <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste' <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required information. Since <br />it cannot be determined how long the containers has been on site, immediately contact a licensed hazardous waste <br />hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br />Violation #608 - Failed to properly manage used oil and fuel filters. <br />Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br />lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters <br />for the last three years, maintain them on site, and submit copies to the EHD. <br />Open violations from September 05, 2013 inspection <br />Page 2 of 2 <br />
