covering the generator activities of the previous calendar year. <br />Violation #4: Within 30 days of receiving this report, document all required training and experience <br />acquired (including initial training and annual training) in personnel records of employees who work <br />with hazardous waste. <br />Violation #5: Within 15 days of receiving this report, you must apply to the State of California <br />Department of Health Services Toxic Substance Control Division for a variance permit which would <br />allow for the treatment of hazardous waste on site. <br />When all violations have been corrected, please send written certification to this office at the letterhead <br />address.
