(Repeat) An emergency coordinator and modified contingency plan information is lacking. There <br /> must be at least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone number of the <br /> emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; <br /> and the phone number of the fire department, unless the facility has a direct alarm. Violation was <br /> corrected. <br /> 999. HSC 25404 (e)(4). Electronic submittal requirement on the California Environmental Reporting <br /> System (CERS) has not been met. Facility shall submit electronic information for hazardous materials <br /> management system, including hazardous waste, on the CERS website, which can be found on http:.// <br /> cers.calepa.ca.gov/. Immediately comply with the CERS electronic submittal requirement per this <br /> section. You may contact EHD to arrange an appointment to assist you toward this compliance. <br /> NOTES <br /> The amount of hazardous waste generated at this facility in 2012 could not be determined since all the <br /> disposal records the last three years were not available for review during this inspection. Facility <br /> Hazardous Waste Generator category shall remain unchanged until the missing hazardous waste <br /> receipts has been submitted and reviewed (see above Item 122/125). <br /> Discussed with generator that a copy of each Hazardous Waste disposal record, drained oil filter and <br /> spent lead acid bill of lading, and universal waste disposal record must be maintained on site for at least <br /> 3 years available for inspection. A copy of Auto Repair/Body Best Management Practice factsheet, <br /> CUPA class schedule, blank hazardous waste labels, hazardous waste labeling factsheet, and San <br /> Joaquin County Small Business Hazardous Waste Disposal Program pamphlet, have been provided to <br /> generator during this inspection. <br /> Discussed with generator that an outstanding balance exists with EHD. Recommended generator to <br /> immediately contact with EHD Accounting Department to resolve this issue.
