A generator shall keep a <br /> record of each shipment of universal waste (fluorescent bulbs, alkaline batteries, electronics, aerosols, <br /> CRTs, mercury containing equipment) on site for at least three years. This record may take the form of <br /> a log, invoice, manifest, bill of lading, or other shipping document, and shall include the following: <br /> - Name and address of destination facility <br /> - Quantity of each type of universal waste <br /> - Date of disposal <br /> Begin this practice immediately. <br /> Complete & submit a copy of the Return to Compliance Certification to the EHD by March 12, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> "Facility generated less than 5 tons of hazardous waste in 2010." <br /> Notes: (corrected on site) <br /> --Six empty 55 gallon blue poly drums that previously contained hydrofluoric acid solution were observed <br /> unlabeled behind the dumpster near the hazardous waste storage area. Containers larger than five <br /> gallons that previously held a hazardous material shall be marked with the date they were emptied and <br /> managed within one year of being emptied. These drums were labeled on site. <br /> -- Two spill pallets under the hazardous waste drums were observed filled to the top with liquid. All <br /> facility communications, alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment, shall be tested and maintained as necessary to assure its proper operation <br /> in time of emergency. Spill pallets shall be maintained clean and dry at all times.
