Hazelton, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. A form is provided that can <br /> be used for this purpose. Discrepancy was corrected on site. <br /> A copy of Body Best Management Practice factsheet, CUPA class schedule, blank hazardous waste <br /> labels, hazardous waste labeling factsheet, and San Joaquin County Small Business Hazardous Waste <br /> Disposal Program pamphlet, have been provided to generator during this inspection. <br /> Discussed with Jaime Universal Waste management requirements. Universal wastes include aerosol <br /> cans, lamps, alkaline batteries, mercury containing switches, CRT, electronic devices, etc. in a closed <br /> container with a method to identify the content and the start date of accumulation. Universal Wastes <br /> must be managed within 1 year of accumulation, and the disposal records maintained on site for at least <br /> 3 years. <br /> Submit the following to EHD by: June 11, 2012 <br /> 1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br /> your records). <br /> 2) A written statement how each violation was corrected <br /> 3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br /> i <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Jeffrey Wong (209) 468-0335 <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
