• 4101 N. HOLLY DRIVE, TRACY is <br />HAZARDOUS WASTE INSPECTION REPORT <br />Violation #21 --Failed to keep containers of hazardous waste closed except when adding or <br />removing hazardous waste. <br />The following containers with hazardous waste were not closed at the time of the inspection: <br />a. One 55 drum and at least one smaller container of oily waste in upstairs "Asphalt <br />Mixing" area <br />b. Two partially -filled 55 drums and at least one 5 -gallon container of oily waste in <br />"Asphalt Containment" area <br />c. Plastic yellow pallets used as secondary containment beneath the oil drums, they are <br />considered primary containment due to storage of oil in them <br />Immediately close all containers storing hazardous waste and keep all containers closed <br />except when adding or removing hazardous waste. <br />Violation #25—Failed to send copy of the manifest to DTSC within 30 days. <br />"Blue" copy of manifest never sent to DTSC by generator for manifest #25126709 <br />dated 6/7/06. <br />Immediately mail blue copy of manifest to DTSC. <br />Violation #33—Failed to label used oil filters as "drained used oil filters" with a date of <br />accumulation. <br />Drained oil filters were not properly stored or labeled. <br />Immediately begin storing used oil filters according to CCR section 66266.130(c). <br />Violation #43, 46 & 47—Failed to provide a contingency plan or emergency coordinator. <br />There was no contingency plan available for review and no designated emergency <br />coordinator for this site. <br />Within 30 days of receiving this report, provide a complete contingency plan to EHD <br />including the onsite emergency coordinator as required. <br />Violation #48—Facility failed to submit a written report to DTSC within 15 days of an <br />incident. <br />Facility had a major asphalt release of hundreds of gallons of asphalt onsite and failed to <br />notify DTSC is writing within 15 days of the release. <br />Within 15 days of any emergency release, the emergency coordinator shall notify DTSC of <br />all required information as required in section CCR 66265.56. <br />Violation #49 & 78 --Personnel training records incomplete. <br />No personnel raining records for personnel handling hazardous waste were available. <br />By 7-13-08, submit a complete personnel training records to EHD for all employees who <br />handle hazardous waste including universal waste. <br />Violation 75 & 76—Failed to properly manage universal waste. <br />Universal waste fluorescent tubes are not stored or labeled pursuant to regulations. <br />Immediately properly label and store all universal waste pursuant to CCR. <br />Violation 91—Failed to mark empty container according to section. <br />At least one empty container in the hazardous waste storage area was lacking the <br />words "empty container" and the date it became empty. <br />Immediately, label all empty containers that previously stored a hazardous material <br />according to the section. <br />EHD Inspector: <br />i <br />Received By: Date Received: / i� Zc'XI-Y <br />
