Simplot Inspection <br /> Tank No. 8 August 27-28,2008 <br /> Lathrop, CA Page 8 of 54 <br /> SUMMARY, REPAIR RECOMMENDATIONS AND POST-REPAIR FINDINGS <br /> (CONT'D.) <br /> NOZZLES AND APPURTENANCES: Nozzle L (4-inch suction) has a weld spacing <br /> between its nozzle neck weld and the shell-to-bottom joint less than that required by API <br /> 650, Figure 5-8. This detail has been present for an extended period and no corrective <br /> action is required at this time. This area should be monitored in the future for signs of <br /> accelerated corrosion due to possible increased stress concentrations. Also, if <br /> modification to this item is planned or if a new bottom is installed above the current <br /> bottom, then plans should be made to modify this detail to meet current API 650 weld <br /> spacing requirements. <br /> Manway A and Nozzle L have reinforcing plates without telltale holes. API 650, Para. <br /> 5.7.2.10 indicates reinforcing plates should be installed with telltale holes. Telltale holes <br /> should be drilled and the space behind each reinforcing plate verified to be gas-free prior <br /> to any future hot work on these nozzles in case any undetected product has leaked into <br /> this area. The telltale holes should then be left open to the atmosphere. <br /> Two (2) internal lap welded patch plates were installed at this time to replace Nozzles I <br /> and J. The patch plates do not meet the requirements of API 653 Para. 9.3.1.6 that <br /> states "repair plates positioned on the shell interior shall be located such that the toe-to- <br /> toe weld clearances are a minimum of 6 in. to the shell-to-bottom weld". It should be <br /> noted that Dye Penetrant (PT) testing was performed by the maintenance contractor on <br /> the lap welded patch plates. <br /> The coating on the shell nozzles is failing or is not coated. These details should be <br /> properly cleaned and re-coated at the next available opportunity. <br /> All nozzle valves should be maintained during this out-of-service period. <br /> FIXED ROOF: Visual (VT) inspection noted numerous holes around the manway and <br /> top fill nozzle. In addition, holes were noted under some of the pipe support pads out on <br /> the roof. These areas should be repaired per API 653, Para. 4.2.1.2 that states "roof <br /> plates corroded to an average thickness of less than 0.09 in. any 100 in.z area or roof <br /> plates with any holes through the roof plate shall be repaired or replaced". <br /> Post-Repair Comments: At the time of this post-repair inspection, all areas of excessive <br /> metal loss on the fixed roof were property repaired with suitable size lap welded patch <br /> plates.
