Submit a <br /> statement and supporting documentation explaining how this waste was managed by 6-3-12. <br /> 404. An open unlabeled five gallon bucket of used oil was found in the shop. All hazardous waste <br /> containers shall be closed at all times except when adding or removing waste. During the inspection <br /> Michael Rogan had this container closed and transferred to the oil room. Continue to ensure all <br /> hazardous waste containers are closed when not adding or removing waste. <br /> 605. In the oil room, two five gallon buckets of oily debris were not marked with a hazardous property. In <br /> the shop an open five gallon bucket of used oil was not labeled. Ag Ray took over business operations in <br /> November 2011. All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> During the inspection, Michael Rogan marked toxic on the two five gallon buckets of oily debris and <br /> completed a hazardous waste label for the unlabeled five gallon bucket of used oil. Since it cannot be <br /> determined how long the unlabeled five gallon bucket of used oil has been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the <br /> manifest to the EHD by 6-3-12. <br /> 804. Twenty two universal waste(UW) lamps were not stored in a closed container. A UW handler shall <br /> manage lamps in a way that prevents releases of any UW or component of a UW to the environment. a <br /> UW handler shall contain any lamp in a container or package that is structurally sound, adequate to <br /> prevent breakage, and compatible with the contents of the lamp. Such a container or package shall <br /> remain closed and shall lack evidence of leakage or damage that could cause leakage under reasonably <br /> foreseeable conditions. During the inspection, Michael Rogan made a cover for the box of UW lamps. <br /> Continue to ensure that UW lamps are stored in a compatible container and close it. <br /> 808. Twenty two UW lamps were not labeled to identify type of UW. A UW handler shall label or mark <br /> UW to identify the type of waste.
