If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Records of Shipment for Universal Waste were not available for review at the time of inspections. A universal waste handler shall keep a record of each shipment of universal waste sent from the uw handler's facility to another facility, The record may be in the form of a log, invoice, manifest, bill of lading or other shipping document. The record shall include the following: 1) The name and address of the universal waste handler or destination facility to which the universal waste was sent 2) The quantity, count or weight, of each type of waste 3) The date of departure of the shipment of universal waste. Immediately begin maintaining records for shipments of universal waste. Note: Bill of ladings must be kept for lead acid batteries if the generator handles or transports 10 or more batteries at once, ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
