To claim an exclusion or exemption from classifying this as a waste, the <br /> following information shall be submitted every two years: <br /> - name„ site address, mailing address, and telephone number of the owner or operator of the facility that recycles <br /> the material; <br /> - name and address of the generator of the recyclable material; <br /> -documentation that the requirements of any exemptions or exclusions pursuant to HSC 25143.2 are met. <br /> Submit statements or documentation showing that above materials meet the recycling exemption of HSC 25143.2 <br /> and which subsection applies. Immediately provide all required information for every recyclable material that you <br /> wish to claim the exclusion or exemption and provide that information every two years. Also, submit the Recyclable <br /> Materials Report in CERS, and maintain recycling records onsite for review during inspection. If the required <br /> information cannot be provided, the materials must immediately be handled as a hazardous waste. <br /> This is a Class II violation. <br /> 117 HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 6-16-2014 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted 30 days from the receipt <br /> of the full inspection report. This information has not been received in its entirety resulting in a non-compliant status <br /> for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that <br /> includes a statement documenting corrective actions taken or proposing corrective actions which will be taken, refer <br /> to the 6-16-2014 inspection and the follow up correspondence for the outstanding violations.
