A 55 gallon red steel drum of used absorbent was observed near the truck wash with an <br /> accumulation start date of June 2010. Facilities who generate 1000 kg or more of hazardous waste per <br /> month and or exceed 6000 kg of waste stored on site at any time may not store waste on site for longer <br /> than 90 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under <br /> manifest and submit a copy of the manifest to the EHD by July 16, 2011. <br /> REPEAT VIOLATION <br /> 20. Three 5 gallon buckets of diesel particulate were observed, two without labels, one with a hazardous <br /> waste label with no other information. A 5 gallon bucket of oily liquid was observed in the wash rack <br /> without a label. A container of used oil was observed in the shop without a label, All hazardous waste <br /> containers shall be marked with the following information.- <br /> 1. <br /> nformation:1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long the unlabeled containers of waste have been on <br /> site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and <br /> submit a copy of the manifest to the EHD by July 16, 2011. <br /> REPEAT VIOLATION <br /> 49. The hazardous waste training program and training records on site were incomplete. Facility <br /> personnel shall complete a training program in accordance with Title 22 regulations. This program shall <br /> be directed by a person trained in hazardous waste management procedures and shall include the <br /> following: <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY 1S SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: R /'�ived�y: Title: <br /> EHD 23-0(O R 08!10110 CONTINUATION FORM <br />
