(continued) <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name & job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a copy of the training program and records the the EHD by July 16, 2011. <br /> REPEAT VIOLATION <br /> 60, 61, & 62. Manifest number 000897928GBF (1/13/10) was found without a signed off copy from the <br /> destination facility. Manifest numbers 000895109SKS (2/19/09), 000777596GBF (315/09), and <br /> 000897727GBF (7/8109) were not found on site. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 35 days of the date <br /> the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the <br /> owner or operator of the designated facility to determine the status of the waste. If the generator did not <br /> receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was <br /> accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br /> copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate <br /> the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to <br /> DTSC. By July 16, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of <br /> the exception report submitted to DTSC.
