The eye wash solution on the wall was observed with an expiration date of January 2011. All facility <br /> communications and alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment, shall be tested and maintained as necessary to assure its proper operation <br /> in time of emergency. Immediately conduct all necessary testing and maintenance on this equipment. <br /> 27. A 5 gallon bucket of oily liquid in the wash rack and a container of used oil in the shop were <br /> observed without lids. All hazardous waste containers shall be closed at all times except when adding <br /> or removing waste. Immediately close these containers and ensure all hazardous waste containers are <br /> closed when not adding or removing waste. <br /> 46. The contingency plan was not available at the time of inspection. A copy of the contingency plan <br /> and all revisions to the plan shall be maintained at the facility at all times. A contingency plan shall <br /> include all of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br /> & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br /> location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used <br /> -telephone number of Cal EMA <br /> This information must be kept current at all times. Immediately locate a copy of the contingency plan for <br /> the facility and submit a copy of the plan to the EHD by July 16, 2011. <br /> 84. A 5 gallon bucket of universal waste alkaline batteries was observed with an accumulation start date <br /> of October 9, 2009. A universal waste generator shall accumulate universal waste for no longer than <br /> one year from the date the waste was generated. Immediately dispose of this waste as universal waste <br /> and submit a copy of the disposal record to the EHD by July 16, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: R ceived By: Title: <br /> -ZL <br /> EHD 23-02-61"8110110 08/10110 CONTINUATION FORM <br />
