Failed to maintain spill control equipment. There is approximately a couple of inches of oily liquid <br /> stored in the secondary containment of used oil tank. Spill control equipment purpose is to catch any <br /> unintentional release of substances but not to store the waste. All facility communications and alarm <br /> systems, fire protection equipment, and decontamination equipment, shall be tested and maintained as <br /> necessary to assure its proper operation in time of emergency. Immediately maintain the secondary <br /> containment empty to assure it proper function is available in time of emergency and to ensure that the <br /> used oil tank structural integrity itself is not compromised. <br /> 122,123,124. Manifest number 000489865WAS shipped 05/31/12 was found without a signed off copy <br /> from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off <br /> by the destination facility on site for three years and have them readily available for review. If the <br /> generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the facility to which the generator's waste was submitted within 60 days of the date the waste was <br /> accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with <br /> some indication that the generator has not received confirmation of delivery. This information shall be <br /> submitted to: <br /> DTSC Report Repository, Generator Information Services Section <br /> P.O. Box 806, Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit the required information to <br /> DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information <br /> submitted to DTSC. <br /> Submit to SJC EHD within 30 days (12/17/12): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. requesting or supporting documentation ie. photos, disposal receipts, signed manifest <br /> 4. one copy of the completed "California Environmental Reporting System Lead User Authorization <br /> Form" to allow EHD authorization to facility's CERS data. Businesses must submit electronic data of <br /> HMBP on a state website starting 2013. <br /> Notes: <br /> - Provided: Haz Waste labeling instructions, CERS info &Authorization form, free training schedule <br /> - QA/QC lab performs fat analysis on cheese. Sulfuric acid is used for the decomposition of fats and is <br /> itself decomposed in the reaction. The end waste of acid becomes neutral pH.
