American Street <br /> Project Contact: Brad Frahm, Seibold Corp. Phone: 948-2152 <br /> * See page 3, scope of work for soil samples and tanks. <br /> 1. Facility Background - Work Plan <br /> To uncover by excavation the previous excavated hole of the 1,000 <br /> gallon diesel tank and retake soil samples at the west end of tank. <br /> We will provide public safety until the job is complete. <br /> 2. Key Personnel <br /> The Job foreman , Brad Frahm, is in charge of all site work <br /> being performed by laborers and equipment operators. Joel Gentles <br /> is the company expeditor and site Safety Officer. All laborers <br /> report to Brad Frahm, and or, Joel Gentles. (29 CFR 1910.120) . <br /> The Contractors phone number is (209) 948-2152. <br /> 3. Job Hazard Analysis <br /> There are no expected health risks at this job site. The route <br /> of entry to the site is from the American St. entrance to this <br /> job site. <br /> 4. Job Hazard Summary <br /> Potential risks to anyone on site are stepping into an open <br /> trench or tank hole. If any vapors are encountered during exca- <br /> vation of soil air purifying respirators (APR) with a set of <br /> organic vapor cartridges will be used by the operator and job <br /> foreman. <br /> 5. Exposure Monitoring Plan <br /> An L.E.L. meter, (Lower Explosion Level Meter) , will be on site <br /> to measure L.E.L. levels and oxygen in the tanks during triple <br /> rinsing and inerting of the tanks. No heat stress is expected <br /> due to the cool weather. Noise levels will be relatively low <br /> due to the use of small equipment on site. Workers are encouraged <br /> to use ear plugs in any case. <br /> 6. Personnel Protective Eauipment <br /> Protective clothing on site includes gloves, hard hats, and <br /> respirators when needed. If respirators are needed we use half <br /> face with (HEPA) Organic Vapor Cartridges. <br /> 7. Site Control <br /> The tank hole and piping area of the job site is completely <br /> fenced off from the public. A two foot area around the tank hole <br />
