Please complete the following items within 30 days of the close of escrow <br />(Health and Safety Code section 25284(b -c). <br />1. Underground Storage Tanks — Facility Form — Provide all current information, sign <br />and date at the bottom. <br />2. Underground Storage Tanks — Tank Forms — Provide all current information, sign <br />and date the bottom of each form <br />3. Certification of Financial Responsibility/Letter from Chief Financial Officer: <br />Complete the forms provided. This document is required to be submitted to this <br />office annually. <br />4. Provide documentation (Notice of Sale or Grant Deed) indicating when escrow <br />closed and name(s) of previous and new owner(s). <br />5. Underground Storage Tank Monitoring and Response plans: Complete and submit <br />the provided forms. <br />6. Report #5021: Update all information for all programs at this site(s) including the <br />UST and Hazardous Waste programs, sign and date. <br />7. Complete and return the form "Owner Statements of Designated Underground <br />Storage Tank (UST) Operator and Understanding of and Compliance with UST <br />Requirements." <br />8. Go to: http://www.waterboards.ca.gov/water_issues/programs/ust to obtain the <br />current statutes (Underground Storage of Hazardous Substances) and UST <br />Regulations that you are required to have for your monitoring and response plans. <br />9. Hazardous Materials Management Plan: Contact the San Joaquin County Office of <br />Emergency Services at (209) 468-3969 for information on updating this plan. <br />10. Go to: http://www.dtsc.ca.gov/IDManifest/index.cfm for information and instructions <br />for obtaining a permanent California EPA ID number: Complete California <br />Department of Toxic Substances Control (DTSC) form 1358 and fax it to (916) 255- <br />4703. <br />Page 1 of 2 <br />
