The Discharger shall comply with the Monitoring and Reporting Program No. 95-097 whichisPAq of this er, and any revisions thereto as ordered by the Executive Officer, <br />�. The Discharger shall submit, by15_.I=e 1995 a work platy and time schedule for <br />implementing a ground water monitoring program for the wastewater disposal fields, The ground <br />water monitoring network shall be sufficient to determine ground water gradient plus the <br />upgradient and downgra&ent water quality beneath the disposal fields. The Discharger shall <br />implement an approved ground water monitoring plan bX 1 Se tember 1995. <br />3. 'The Discharger shall submit, by 15 June 1995, an operations and Maintenance Pladfor the <br />wastewater application to the disposal fields. The plan must address op mdoa and maintemamce <br />of wastewater disposal activities for both the-irrigafmg and non -irrigating months. <br />4. The Discharger shall comply with the "Standard Provisions and Reporting kequirements for <br />Waste. Discharge Requirements", dated 1 March 1991, which arc attached hereto and by rafcrmce. <br />a part of this Ordcr.
