Sacramento St. Lodi, CA <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II, or MINOR -Notice to Com <br />accumulation area and marking or labeling the area to identify the earliest date that any UW in the area <br />became a waste or was received; or (6) Any other method which clearly demonstrates the length of time <br />that the UW has been accumulated from the date it became a waste or was received. Correct <br />immediately by demonstrating when the UW lamps became a UW. Submit a statement and supporting <br />documentation explaining how this waste was managed by December 19, 2012. <br />305. Rags and used oil were found in the secondary containment spill control equipment of the used oil <br />tank. All facility communications or alarm systems, fire protection equipment, spill control equipment, <br />and decontamination equipment, where required, shall be tested and maintained as necessary to assure <br />its proper operation in time of emergency. Correct immediately by removing the liquid, make a Title 22 <br />hazardous waste determination, and manage according to Title 22 hazardous waste regulations. Submit <br />a statement and supporting documentation explaining how this waste was managed by December 19, <br />2012. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by December 19, 2012. <br />Note: The following first time issue was corrected during the inspection. <br />703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and post the required information by a phone. A form is provided that can be <br />used for this purpose.
