Correct immediately by demonstrating when the UW lamps became a UW. Submit a statement and supporting documentation explaining how this waste was managed by December 19, 2012. 305. Rags and used oil were found in the secondary containment spill control equipment of the used oil tank. All facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of emergency. Correct immediately by removing the liquid, make a Title 22 hazardous waste determination, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed by December 19, 2012. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by December 19, 2012. Note: The following first time issue was corrected during the inspection. 703. An emergency coordinator and modified contingency plan information is lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form is provided that can be used for this purpose. John Koch completed the form and posted it next to the facility phone during the inspection. A copy of the California Electronic Reporting System information sheet and Lead User form were provided to John Koch. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
