Manifest numbers 002571190GBF,00198491 GBF, and 002739094GBF were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. This is a Class II violation. 301 CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. The following was noted at the time of inspection: 1) One leaking fork lift(with some absorbent below it)was noted outside, in the vicinity of the diesel fuel tank(located on the southwest portion of the property). 2) Several pieces of I leaking machinery (which was being repaired) were also noted inside the shop area. 3)Oily ground with some applied absorbent was noted near the waste oil tank, located inside the indoor hazardous waste storage area. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately fix all leaks and clean all the staining and manage all clean up debris according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. This is a Class II violation. 302 CCR 66265.32 Failed to maintain required emergency equipment. An eye wash station located inside the hazardous waste storage area had an expired eye wash solution,which expired on 3/2011. All facilities shall be equipped with an internal communication or alarm system, a device capable of calling outside emergency help, spill control equipment, and decontamination equipment at all times. Immediately provide all necessary decontamination equipment and maintain it on site. Submit proof of correction to the EHD. This is a Class II violation. Page 8 of 12
