Stockton Plant <br /> this waste will be transported under manifest <br /> as a hazardous waste. <br /> Violation #4 : No OPMR waste has been shipped since <br /> notification by the Public Health Services of <br /> the hazardous nature of this waste. Should <br /> characterization not be completed within the <br /> 90 day holding time for hazardous wastes, <br /> this waste will be shipped as a hazardous <br /> waste with all labeling in accordance with <br /> the applicable Department of Transportation <br /> regulations. <br /> Violation #5: All OPMR waste drums have been labeled as <br /> hazardous waste beginning September 8 , 1993 <br /> upon notification by the Public Health <br /> Services of the hazardous nature of this <br /> waste. <br /> Violation #6: An eyewash system was installed in the <br /> hazardous waste storage area, as requested <br /> during the inspection, on August 13 , 1993 . <br /> I certify under penalty of law that this document and all <br /> attachments were prepared under my direction or supervision in <br /> accordance with a system designed to assure that qualified <br /> personnel properly gather and evaluate the information submitted. <br /> Based on my inquiry of the person or persons directly responsible <br /> for gathering the information, the information submitted is, to <br /> the best of my knowledge and belief, true, accurate and complete. <br /> I am aware that there are significant penalties for submitting <br /> false information, including the possibility of fine and <br /> imprisonment for knowing violations. <br /> sincerely, , <br /> i <br /> -Are <br /> Phoebe C.
