H. SCHEDULE of CORRECTION <br /> Violations #1 & #2: <br /> Effective immediately, all drums containing hazardous waste shall be labeled with the following <br /> information. Drums must be secured between waste additions and labeled as follows: <br /> a) The accumulation start date. <br /> b) The words "Hazardous Waste". <br /> C) Composition and physical state of the waste. <br /> d) Statement or statements which call attention to the particular hazardous properties of the <br /> waste. <br /> e) Your name and address. <br /> Violation #3: <br /> Within 30 days of receipt of this report Dopaco must institute an appropriate "Health & Safety <br /> Training Program" for their employees. <br /> When all violation have been corrected,please send written certification to this office at the letterhead address. <br /> PHS-EHD will conduct a follow-up inspection of the facility to verify compliance. <br /> The issuance of this report of violation and schedule of correction does not preclude PHS-EHD from taking <br /> any administrative, civil or criminal action as a result of the violation noted herein. <br /> If you should have any questions or should require additional information, contact Carol Oz, R.E.H.S., at <br /> (209)4683468. <br /> Jogi Khanna, M.D., M.P.H. <br /> Health Officer <br /> Ronald Valinoti, Director <br /> Environmental Health Division <br /> c: Mr. Chuck McLaughlin <br /> DHS-TSCP S & E UNIT <br />
