SANJ O A Q U I N Environmental Health Department <br /> —COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 12/14/2017 to <br /> 12/28/2017 for C00045277 located at <br /> 2121 N ZUCKERMAN RD <br /> Activity Date Recorded by <br /> 12/21/2017 Received a call from Ralph Roberts of PG&E, who provided an update on the PE 2546 FLORIDO <br /> 60 min production fluid clean up. He said that the release was caused by an underground SC 544 <br /> line failure. The material is no longer releasing and the pipe has been isolated. No <br /> waterways or storm drains were impacted. PG&E is investigating how and when <br /> they will begin excavation. I told Ralph that I would relay this information to my <br /> supervisors, Muni N. and Garret B., and I would get back to him on what the EHD <br /> plans to do-either verify the clean up or refer the clean up to the State Waterboard. <br /> I told Ralph to keep me updated. Ralp agreed. <br /> After discussion with Muni and Garrett, I called Steven Meeks of the State <br /> Waterboard and asked if he would be willing to accept the clean up referral. <br /> Steven accepted, and told me to send all the information I had on the release to <br /> Mark Bare (mark.bare@waterboards.ca.gov) and to CC <br /> seven.meeks@waterboards.ca.gov in the email. I agreed and told Steven I would <br /> let Ralph Roberts know that the clean up is being referred to the State <br /> Waterboard. <br /> I called Ralph to let him know I spoke to Steven Meeks and that the Waterboard <br /> would now be overseeing the cleanup. Ralph understood and told me that PG&E <br /> is planning to have PSC begin excavation about a foot deep into the impacted soil <br /> near the damaged piping. Ralph said he would call USA North before continuing <br /> the excavation, which would likely continue on 12/27/17. 1 told Ralph that if they <br /> are able to get PSC to the site today to start the excavation, I would try to make it <br /> to the site to assess the issue. Ralph agreed and will keep me updated. <br /> Ralph emailed me that PSC will not be able to start the excvation until 12/27/17, <br /> once the USA clears. I replied, telling Ralph I would notify the State Waterboard in <br /> case they want to send one of there staff members to witness the beginning of the <br /> excavation. I also asked Ralph to email me the analytical results for the soil <br /> samples that were taken on 12/14/17. <br /> 12/21/2017 Downloaded photos and began creating a photo log. PE 2546 FLORIDO <br /> 12 min SC 544 <br /> 12/22/2017 Completed photo logs from photos taken during 12/14/17 and 12/15/17 PE 2546 FLORIDO <br /> so min inspections. SC 544 <br /> 12/22/2017 Revised and completed the Emergency Response Record Form. Created PE 2546 FLORIDO <br /> 120 min reference letter to State Waterboard's Mark Bare and Steven Meeks. Printed and SC 007 <br /> gathered all spill complaint documentation, and submitted to Garrett Backus for <br /> his review. <br /> 12/27/2017 Received an email from Ralph Roberts saying that they hope to be able to PE 2546 FLORIDO <br /> 24 min excavate to investigate the piping this afternoon. He also provided the initial soil SC 002 <br /> results taken just before they started minor surface excavation in the areas they <br /> though were most impacted around the tank containments. Ralph said these <br /> spoils would have ended up in the roll off bin for disposal as non hazardous waste. <br /> I printed and date stamped the soil results, but have yet to review them. <br /> I discussed the spill clean up with Muni. Muni said he is going to forward me an <br /> email he received from the State Waterboard regarding this spill clean up. <br /> Page 3 of 4 <br />
