Within 15 days after the <br /> incident the administrator(or designee) shall SUbIllit H written report on the illCident <br /> to the I)ci))1rtment of 1-lealth Services. The report shall include: <br /> • Name, address, and telephone nUmber of the administrator (or designee) <br /> • Name, address, and telephone 1ll11I1bc1- 01'01C ttacility <br /> • Date, lime, and type of incident (e.g., fire, explosion) <br /> • Name and quantity of nnaterlal(s) Involved <br /> • The extent of'Inf uries it anv <br /> An assessment of actual or potential hazards to human health lar the environment, <br /> where this is applicable and. l-stimated (ILlantity and disposition of recovered material <br /> that resulted from the incident. <br /> Shill / Release: <br /> Procedures will outline responsibilities to be assumed by those in the Medical facility <br /> and will specifically address the role of external agencies contracted to address <br /> emergency spills. <br /> • Spills mtlst be cleaned up immediately by a properly trained person using <br /> appropriate personal protective equipment in relation to the spill. <br /> • Spills are cleaned according to Hazardous Waste Management Policy <br /> EC.HI.056: Spill Managentent: Hazardous Materials & Waste. <br /> • In the event of an employee exposure, the ernployee shall immediately be <br /> evaluated at the; Employee Health Services or an Emergency Department. <br /> When a Small spill is identified the following process will be rollowed: <br /> Staffthat discovers the spill will follow SIX: <br /> S = Safety First? Clear the area of all non-essential personal. <br /> I = Isolate the area by closing doors/ windows. <br /> N= Notily your Manager f Supervisor and alert the departmental spill response team <br /> for clean-up <br /> Radiation: <br /> We only have Isotopes. ProCedUres for Radiation accidents are to be followed as <br /> indicated by the Radiation Safety Officer and Nuclear Medicine Policy 1.03- <br /> Radiation Emergency Procedures and 1.04-Radiation Safety. <br /> Fire / Explosions: <br /> Procedures for fire are to be followed as indicated in the Life Safety Policy <br /> EC.LS.020-Fire RcspD11Ne Plan- Medical ("enters or EC.LS.020A-[sire Response <br /> Plan-Medical Offices. <br />
