Hazardous Materials & Waste Contingency Pian <br /> Kaiser Permanente Stockton Medical Offices <br /> 1/01/17-i2/31/17 SEP 19 2017 <br /> ENVIRONMENTAL HEALTh <br /> (e) Describe your hazardous waste emergency action plan, including; USMYrt•' A <br /> for handling spills, exposures, equipment failures, etc. <br /> The Contingency Plan shall be carried out immediately whenever there is a fire.. <br /> explosion, spill or release of hazardous rnaterial/waste or hazardous material/ waste <br /> constituents to the air, soil or Surface water which could threaten human health or the <br /> environnient. This plan minimizes the hazards to human health and the environment <br /> that may occur as a result of emergencies involving hazardous materials/wastes. The <br /> regulations that specify the contingency plan requirements are found in the Cali lornia <br /> Code or Regulations (CCR), Title 22, Chapter 15, Article 4. beginning with <br /> §66265.50. <br /> As a generator that stores hazardous waste on-site. we are required to complete the <br /> contingency plan. We are to keep one copy of*the plan and submit a copy of the plan <br /> to each agency that may provide emergency response to our facilities. The plan must <br /> be revised whenever the plan !:ails during an emergency, the facility changes, the <br /> contents of the plan change or the regulations change. <br /> The Emergency Coordinator (Safety Officer) has authority to implement the <br /> contingency plan. In the event of ail emergency, the 161lowing personnel shall take <br /> responsibility as Emergency Coordinator (Appendix A and B). In the absence of the <br /> Emergency Coordinator, the Administrator-On-C:all is responsible. <br /> The cell phone number of those assigned as Emergency Coordinators will be <br /> maintained on file in the Operator's Directory. Whenever there is an imminent or <br /> actual emergency situation, the emergency coordinator (or designee) shall <br /> immediately: <br /> e Activate internal facility alarms or CornmUniCatrnn systems, where applicable, to <br /> notify all facility personnel: and <br /> • Notify appropriate State and local agencies with designated response roles if their <br /> help is needed. <br /> Whenever there is a release through spill, lire, or explosion, the emergency <br /> coordinator shall immediately identify the character, exact source, amount, and extent <br /> of any released materials. The emergency coordinator may do this by observation or <br /> review of the facility records or manifests and. ifnecessary, by chemical analysis. <br /> Concurrently, the emergency coordinator shall assess possible hazards to human <br /> health or the environment that may result from the release, fire. or explosion. This <br /> assessment shall consider both direct and indirect effects of release. fic, or explosion <br /> (e.g., the effects of any toxic. irritation, or asphyxiating gases that are generated, or <br /> the effects of any hazardOLIS surface water run-offs from water or chemical agents <br /> used to control fire and heat-induced explosions). If the emergency coordinator <br /> determines that the facility, has had a release, fire or explosion which could threaten <br /> human health, or the environment, outside the facility, the emergency coordinator <br /> Shall report the findings as follows: <br />
