Donnya Heyward, Outpatient Pharmacy Manager, stated that the facility generates Warfarin containers when the medication is dispensed and that these containers are placed in the regular trash, not a hazardous waste container. Warfarin medication that is>0.3%active ingredient is a RCRA P-listed hazardous waste (13001)when it becomes a waste. Waste medication and storage containers which touch the medication are P-listed hazardous waste. The disposal records from the last three years did not show hazardous waste disposal of P001 listed RCRA waste. Hazardous wastes shall be disposed of only by transportation to a permitted hazardous waste treatment, storage, and disposal facility(TSDF). Immediately cease disposal of hazardous waste to the regular trash (Class 3 landfill)and ensure that all future waste is hauled by a licensed hazardous waste transporter to a permitted TSDF. Provide a list of medications the facility dispenses which will be P-listed RCRA hazardous waste if the medication or containers are disposed of by the facility. Provide a statement explaining how P-listed RCRA hazardous waste will be handled at the facility. This is a Class I violation.
