A white 5 gallon container of used compressor tank parts did not list the physical state on the label. <br /> 2.A white 5 gallon container of used oil filters(labeled as hazardous waste)did not list the hazardous property. <br /> 3. In the Allergy Injection Center, two black eight gallon container of used flu vaccine, syringes/needles did not list the <br /> hazardous properties or physical state on the label. <br /> 4. In the first floor Pharmacy, a white container with used empty Quick Care containers did not have hazardous <br /> property or physical state listed on the label. <br /> 5. In the Dermatology area, an one gallon container of used Caritharidin did not have the hazardous property listed on <br /> the label. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> All hazardous labels were corrected during the inspection. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> This inspection report, revised 6-3-15, supersedes all previous versions. Complete and submit a copy of the <br /> Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have <br /> been or will be taken for each violation, and any supporting paperwork, by July 6, 2015 . <br /> NOTES: <br /> 1. A 55 gallon orange container of used oil was open during time of inspection. This was corrected on site. <br /> 2.
