SAFETY MANAGEMENT PROGRAM POLICY/PROCEDURE MANUAL <br /> • TITLE: HAZARDOUS WASTE CONTINGENCY PLAN POLICY NUMBER 5.06 <br /> PAGE 8 OF 14 <br /> 3.4.6 In the event a hazardous waste spill should get into the storm drains, <br /> immediate action shall be taken to stop further contamination. The local <br /> water district and the local Department of Fish and Game shall be notified <br /> immediately for corrective action: <br /> 3.4.7 In the event hazardous waste should spill onto the ground or we should <br /> have an underground tank leak, immediate action shall be taken to stop <br /> further contamination. Stockton Public Utilities, Water Department, and <br /> the local EPA office shall be notified immediately for corrective action. It <br /> may become necessary to contract with an outside hazardous materials <br /> team to aid in soil containment. <br /> 3.5 Clean up of hazardous materials spills. <br /> 3.5.4 Procedures for clean up are to be followed as indicated in the Kaiser <br /> Permanente Medical Care Program - Safety Management - plan, <br /> emergency spill response <br /> • 3.6 Radiation Accidents <br /> 3.6.4 Procedures for Radiation accidents are to be followed as indicated in the <br /> Kaiser Permanente Medical Care Program - Safety Management plan, <br /> Radiation Accident Disaster Plan. <br /> 3.7 Fire or Explosions <br /> 3.7.4 Procedures for fie are to be followed as indicated in the Kaiser <br /> Permanente Medical Care Program - Safety Management Plan under the <br /> Emergency Preparedness Management Plan including: Discovery of a Fire <br /> and Response to the Alarm, Extended Evacuation <br /> 3.7.5 Procedures for explosions are to be followed as indicated in the Kaiser <br /> Permanente Medical Care Program - Safety Management Plan under the <br /> Safety Management Plan including Code Phoenix. <br /> 3.8 Emergency Coordination: <br /> 3.8.4 In the event of an emergency, the following personnel shall take <br /> responsibility as Emergency Coordinator: <br /> • Sharon Likely, Acting Emergency Coordinator, 7373 West Lane, <br /> Stockton, CA (209) 476 3212 <br /> • 24 hour pager (209) 546 1230 <br /> Fresno Revision dates: 09/12/96, 6/25/98, 3/30/00, 8/l/01,Stockton revision date 07/22/02, <br /> 09/19/02, 3/20/03 <br />
