The following are Class II Repeat violations: #605, 804,808,809, and the following are Minor Notice to Comply: 101,122,123,124, 125 101. This facility has an active EPA ID CAL92079674, but has been using an inactive EPA ID number CAL000076770. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately begin using the active EPA ID, or contact DTSC to reactivate your inactive EPA ID number if you choose to continue to use the latter. 122, 123, & 124. Manifest number 002286326SKS (from 2010) and 002663517SKS (from 2011) were not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository, Generator Information Services Section P.O. Box 806, Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. By 1/10/2013, submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
