Copies of the hazardous waste disposal records between April and August for 2011 were not <br /> found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal <br /> facility and all receipts used in a consolidated manifesting procedure on site for three years and have <br /> them readily available for review. Immediately locate a copy of all manifests and receipts for the last <br /> three years, maintain them on site, and submit copies all hazardous waste disposal records for 2011 to <br /> the EHD by 1/10/2013. <br /> 605. (Repeat) No accumulation start date on the hazardous waste used oil. All hazardous waste tanks <br /> shall be marked with the words "Hazardous Waste" and the accumulation start date. Immediately <br /> ensure that all hazardous waste tanks are marked with the required information. Generator provided <br /> information on the recent disposal of the content of the tank, and marked the start date on the label <br /> during this inspection. Violation was corrected on site. <br /> 605. (Repeat) No accumulation start date on a 55-gallon drum of contaminated water and a 55-gallon <br /> drum of waste anti-freeze. All hazardous waste containers shall be marked with the following <br /> information: <br /> 1. the words "Hazardous Waste' <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Generator provided information on the recent disposal of the content of the above <br /> containers, and marked the start date on the label during this inspection. Violation was corrected on <br /> site. <br /> 804 & 808 & 809 (Repeat). Unable to determine the length of storage of at least 11 Universal Waste <br /> (UW) fluorescent lamps stored in two open containers with no start date of accumulation. Universal <br /> Wastes, including aerosol cans, lamps, alkaline batteries, mercury containing switches, CRT, electronic <br /> devices, etc. shall be stored in a closed container that prevents breakage and release to the <br /> environment, labeled to identify the type of Universal Waste, demonstrate the initial date of <br /> accumulation, managed within one year of storage, and retained a copy of the disposal receipt for at <br /> least 3 years on site. Immediately dispose of the above Universal Waste - lamps and submit a copy of <br /> the disposal receipt to EHD by 1/10/2013 and manage all future Universal Waste per this section. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
