Three boxes of universal waste(UW) lamps were left open, A UW handler shall manage lamps in a <br /> way that prevents releases of any UW or component of a UW to the environment, as follows: A UW <br /> handler shall contain any lamp in a container or package that is structurally sound, adequate to prevent <br /> breakage, and compatible with the contents of the lamp. Such a container or package shall remain <br /> closed and shall lack evidence of leakage, spillage or damage that could cause leakage under <br /> reasonably foreseeable conditions. Correct immediately by maintaining UW lamps in a closed container. <br /> 808. The two boxes of UW electronics and accumulation of UW in the storage room are not labeled. A <br /> UW handler shall label or mark UW to identify the type of UW. Electronic devices or a container or pallet <br /> in Or on which the electronic devices are contained, shall be labeled or marked clearly with the following <br /> phrase: "Universal Waste-Electronic Device(s)". Correct immediately by ensuring that all UW is labeled <br /> properly. <br /> 809. The facility did not demonstrate when the UW electronics became a UW. A UW handler shall be <br /> able to demonstrate the length of time that the UW has been accumulated from the date it became a <br /> waste or was received. Correct immediately by demonstrating the length of time that all UW on site has <br /> been accumulated from the date it became a waste or was received. <br /> 812. Ron Browning approximated that UW electronics are disposed once a year, but that he does not <br /> have any copies of disposal records from the UW collection events. A UW handler shall keep a record of <br /> each shipment of UW sent from the UW handler's facility to another facility. The record may take the <br /> form of a log, invoice, manifest, bill of lading or other shipping document. The record for each shipment <br /> of UW sent shall include the following information: (1) The name and address of the UW handler or <br /> destination facility to which the UW was sent; (2) The quantity, count, or weight of each type of UW sent <br /> (e.g., batteries, thermostats, lamps, electronic devices, CRTS's, CRT glass); (3) The date of departure of <br /> the shipment of UW. UW disposal records shall be maintained for three years. Correct immediately by <br /> implementing this practice. <br /> Note: The following issue was corrected during the inspection. <br /> 404. The 55 gallon drum of used oil was left open. All hazardous waste containers shall be closed at all <br /> times except when adding or removing waste. During the inspection the employee closed the used oil <br /> drum.
