The following is an itemized list of hazardous waste violations that have not been <br /> addressed for CABRAL CHRYSLER JEEP DODGE as of February 12, 2016. <br /> Open violations from April 27, 2015 inspection <br /> Violation#107 -Contingency plan is incomplete. <br /> The facility's contingency plan is incomplete. The emergency coordinator information was lacking. The contingency <br /> plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden <br /> release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers(office(hometcell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and t <br /> others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfil <br /> the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> Violation#110 Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> At the time of inspection, Mr. Cohoon stated that only a few of the staff and not all of the technicians who handle <br /> hazardous waste received the required hazardous waste training. Personnel who handle hazardous waste shall be <br /> trained within six months of assignment and take part in an annual review of the initial training received. At a <br /> minimum, the training program shall be designed to ensure that facility personnel are able to respond effectively to <br /> emergencies by familiarizing them with emergency procedures, emergency equipment, and emergency systems, <br /> including where applicable: <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents; and <br /> 6. shutdown of operations. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to eact <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employee <br /> shall be kept for at least three years from the date the employee last worked at the facility.
