The following is an itemized list of hazardous waste violations that have not been <br /> addressed for CABRAL CHRYSLER JEEP DODGE as of February 12, 2016. <br /> Open violations from April 27, 2015 inspection <br /> Violation#111 -Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste were incomplete. The the job title for each positi <br /> at the facility related to hazardous waste management and the name of each employee filling each job was as well a: <br /> the written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions were lacking. <br /> Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to eact <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employe( <br /> shall be kept for at least three years from the date the employee last worked at the facility. Ensure that employee <br /> training is properly documented. <br /> Violation#301 -Facility not maintained to minimize the release of a hazardous waste. <br /> Metal fines were observed on the floor around the brake pad grinding equipment. Oily staining was noted beneath th, <br /> cardboard box located in the corner of the outdoor car lift area; this box contained various open containers of <br /> automotive fluids (various amounts of oily liquids were noted inside these containers, some of which lacked caps).Oi <br /> staining was also noted around the drain pan discussed previously in this report and also located in the outdoor lift <br /> area. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of <br /> hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately <br /> containerize all hazardous wastes, clean the aforementioned areas, and manage all generated wastes according to <br /> Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste <br /> was managed. <br /> Violation #303 -Failed to maintain all communication or alarm systems,spill control, or decontamination <br /> equipment. <br /> Reddish liquid was noted inside the secondary containment pallet located inside the hazardous waste storage area. <br /> All facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Maintain all secondary containment empty and free of liquids, submit proof of correction to the EHD. <br /> Violation #304-Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> One 55-gallon hazardous waste container, located in the hazardous waste storage room, was positioned in a way th, <br /> disallowed its unobstructed inspection. Adequate aisle space shall be maintained to allow the unobstructed <br /> movement of personnel, fire protection, spill control equipment, and decontamination equipment to all areas of facilit, <br /> operation in an emergency. Immediately maintain aisle space to allow the unobstructed movement of personnel, fire <br /> protection, spill control equipment, and decontamination equipment to any area of facility operation in an emergency. <br /> Violation#403- Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> One drain pan containing dark oily liquid was observed in the outdoor car lift area. Mr. Cohoon stated that it should <br /> not be there and has been likely left behind and forgotten after an oil change. All hazardous waste containers shall bf <br /> closed at all times except when adding or removing waste. Immediately containerize this waste and ensure all <br /> hazardous waste containers are closed when not adding or removing waste and manage according to Title 22 <br /> hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was <br /> managed. <br /> Page 2 of 3 <br />
