SA NUJ 0 AQ U IN Environmental Health Department <br /> OUNTY <br /> Complaint Record Daily Time and Activity Summary from 1/1/2014 to <br /> 6/30/2222 for C00040958 located at <br /> 1780 E YOSEMITE AVE <br /> Activity Date Recorded by <br /> 02/05/2016 Onsite in response to a complaint, a routine facility inspection for PRO540768 was PE 2220 MANZO <br /> also conducted today: SC 004 <br /> Arrived onsite and met with Mr. Barry Walker, I indicated that I was onsite in <br /> response to a complaint for the potential disposal of chemicals into the onsite solid <br /> waste container. Obtained a permission for the inspection, pictures and samples. <br /> Conducted a site walk through accompanied by Mr. Barry Walker. Observed a <br /> fenced in outdoor area attached to the back of the retail store(north of the retail <br /> store).One 1,000 gallon Convault double wall tank(500 gallon gasoline and 500 <br /> gallon diesel compartments)was observed inside the enclosure. Mr. Walker <br /> indicated that he doesn't use the diesel side only the gas and he is thinking of <br /> discontinuing its use due to high cost. Inside this enclosure also observed pallets <br /> of 5 gallon containers of tablet chlorine(Pro Guard) and 1-gallon containers of <br /> muriatic acid which were contained (4 containers each) inside orange totes (these <br /> totes appeared solid and had no visible holes). No leakage was noted inside this <br /> enclosure area, some evidence of recent sweeping and a dust pile were noted <br /> inside the enclosure, the dust pile didn't appear to have any discoloration or <br /> staining. Mr. Walker didn't unlock the enclosure area and all observations were <br /> made from outside of the chain link fence. When I asked Mr. Walker what he does <br /> when bottles of muriatic acid brake, Mr. Walker stated that"if breaks run water <br /> over it and dilute". I asked what kind of containers are being thrown away. Mr. <br /> Walker stated that all empty containers of muriatic acid are sent back to the <br /> supplier and they get$20 credit for sending them back, and empty 5-gallon <br /> container which previously held chlorine tablets are being thrown into solid waste <br /> trash. He also added that sometimes people want to buy the empty chlorine tablet <br /> containers since he sells them for a few dollars and they have a lid and Home <br /> Depot sells them for more without a lid. He also added that he doesn't throw away <br /> his chemicals because they worth money and he can sell them. Next I inspected <br /> the two onsite solid waste containers, one held items like card board and had no <br /> notable waste or odor, the second container had an assortment of lunch related <br /> items, several white trash bags, a large pile of decaying leaves, four observable <br /> 5-gallon containers and some other unidentifiable paper bags. Mr. Walker <br /> indicated that his workers bring leaves and other debris which are removed from <br /> the residential pools(during pool service) and place them inside the garbage at <br /> this location. Some white powder was noted on the outside of the garbage <br /> container on top of one of the loading arms. I asked his if he knew what it was, Mr. <br /> Walker didn't answer conclusively. I noted a strong chlorine odor and told Mr. <br /> Walker that I smell chlorine. Mr. Walker said that"based on SDSs you can throw <br /> away some of the chemicals..", "I fought it out with the fire department that you <br /> can throw some out." I asked if he wouldn't mind removing one of the 5-gallon <br /> containers out, Mr. Walker indicated that he would and went to retrieve a long <br /> wooden stick. The container appeared heavy I asked if he would not mind getting <br /> another tool he can use to remove the container. Mr. Walker went inside and <br /> brought back a broom which had an adequately shaped handle, which Mr. Walker <br /> used to retrieve the container. Mr. Walker opened the container, the container was <br /> %full with wet leaves, some white granules were visible on the side of the <br /> container, also one white disk(approximately 4 inches in diameter)which <br /> appeared to be a bottom of some container had at least% inch accumulation of <br /> white granules in it I took a picture and Mr. Walker deposited the white disk <br /> inside the solid waste container, he also deposited the 5-gallon container inside <br /> the garbage; I was not able to collect any sample from this container. I asked if he <br /> would not mind removing another container for me so we can open it, he stated <br /> that he didn't think that he wanted to do that. Mr. Walker did allow me to collected <br /> a sample from the white powder located on the outside of the trash container. I <br /> Page 2 of 4 <br />
