The Entry Supervisor is capable of anticipating, <br /> recognizing, and evaluating employee exposure to dangerous atmospheres <br /> or other unsafe conditions and is proficient in the use of atmospheric testing <br /> instruments required of the particular entry. <br /> 8. Hazardous Atmosphere: Any of the following conditions that may occur in <br /> normal operations or emergencies. <br /> • Oxygen enrichment (greater than 23.5% 02). <br /> • Oxygen deficiency (less than 19.5% 02). <br /> • Concentrations of contaminants at or above the threshold limit <br /> value or permissible exposure level. <br /> • Concentrations of flammable gases and vapors at or above 10% <br /> of the lower explosive limit (LEL). <br /> • Airborne combustible dust at a concentration at or above its LEL. <br /> • Unknown concentration of air contaminant(s). <br /> 9. Hot Work Permit: Any work involving burning, welding, cutting, or similar <br /> fire or spark producing operations as well as work which produces a source <br /> of ignition, such as drilling, grinding, abrasive blasting, photo equipment, <br /> flashlights, and space heating requires authorization by Hot Work Permit. <br /> 10. Immediately Dangerous to Life and Health: Any condition that poses an <br /> immediate or delayed threat to life, or that would cause irreversible adverse <br /> health effects, or that would interfere with an individual's ability to escape <br /> unaided from a permit space. <br /> 1 I Other Confined Space: An area where existing ventilation in insufficient to <br /> remove dangerous air contamination, or where access or egress is difficult. <br /> Examples of this type of confined space are trenches and excavations. <br /> Training and permitting procedures vary from permit required confined <br /> spaces. <br /> 12. Project Safety Officer: PACIFIC employee who is responsible for the <br /> planning and completion of any job which may include permit-required <br /> confined space entry by a PACIFIC employee or contractor employee. <br /> ADMINIS/CONSP 4 June 2, 1994 <br />
