IV. RESPONSIBILITY <br /> 1. All employees and contractors who work in, about, and in connection with <br /> confined spaces are responsible for complying with this standard. <br /> 2. Supervisors/managers are responsible for ensuring that their employees who <br /> work in, about, and in connection with confined spaces comply with this <br /> standard. <br /> 3. The Project Safety Officer shall issue the permits for the Program. <br /> 4. The Entry Supervisor shall verify that all permit requirements for a specific <br /> permit-required confined space entry have been met. The Entry Supervisor shall <br /> be present at and authorize each entry into a confined space. <br /> 5.
