VI. EQUIPMENT REQUIREMENTS <br /> 1. Only respirators approved by the National Institute for Occupational Safety and <br /> Health are to be selected for use. <br /> • Don and test all respiratory equipment prior to entry into a confined space <br /> so that any failures or problems can be discovered in a safe atmosphere. <br /> 2. An approved safety belt with an attached line shall be provided for each <br /> employee entering a confined space. <br /> • The safety belt shall be the harness type that suspends a person in an upright <br /> position. <br /> • The free end of the line shall be secured outside the entry opening. <br /> • The line shall be at least 1/2-inch diameter and 2,000-pound test. <br /> 3. If required, equipment shall be provided to allow ready entry and exit from a <br /> permit-required confined space. <br /> • A secured entry and exit ladder shall be provided, if required. <br /> • A retrieval system shall be provided for lifting employees out of the space <br /> when entry is made through a top opening. <br /> 4. A monitor shall be provided to test for the existence of/or development of an <br /> oxygen deficient(<19.5%), oxygen enriched (>23.5%), or flammable atmosphere <br /> (>10% LEL). <br /> • The monitor shall audibly alarm whenever the oxygen levels fall outside <br /> the normal range of 19.5%to 23.5% or exceeds 10% of the LEL. <br /> • The monitor shall be capable of monitoring continuously during a permit <br /> space entry. <br /> ADMIMS/CONSP 8 June 2, 1994 <br />
