y <br /> • The monitor shall be able to test the atmosphere at the bottom, middle, <br /> and top from outside of the confined space. <br /> • A means of measuring and/or testing for atmosphere contaminants which <br /> may be present or produced in a permit-required confined space entry <br /> shall be provided. <br /> • When testing for atmospheric hazards, test first for oxygen, then for <br /> combustible gases, and then for atmospheric contaminants. <br /> • Only approved (Class I, D2) lighting and electrical equipment shall be <br /> used in permit-required confined spaces when LEL is >5%. <br /> • Employees shall be provided with, and required to wear, personal <br /> protective equipment when substances corrosive to the skin or capable <br /> of being absorbed through the skin are present or were last contained in <br /> the permit-required confined space. <br /> • Effective means of communication shall be provided to the Attendant. <br /> • A phone or radio shall be provided for alerting emergency response <br /> personnel in case of emergency. <br /> • A system for communication shall be used between employees inside a <br /> permit space and the standby whenever communication is difficult due to <br /> the use of respirators or the employees are not in direct line of sight and <br /> out of audible range due to the nature of the space. <br /> • Appropriate ventilation equipment shall be provided for purging spaces <br /> prior to and during entry into a permit-required confined space.
